TC Pedal Shaft

Would someone please tell me how the pedal shaft is held in place on a TC? The shaft on my car has 2 small holes for a cotter pin approximately 1/4" from the end of the shaft. How are you suppose to get the cotter pin in the hole on the end of the shaft that's inside the chassis rail? There is a rectangular hole in the frame boxing plate approx 2 1/2" in front of the shaft that might be used to slip a cotter pin in with a long needle nose pliers but I'm doubting that's the way they did it back in the days. What am I missing?
